To establish an account with Omnicell please complete the following steps:


1. New Account Information/Credit Application- Complete this form to the best of your knowledge and Sign & Date the bottom.

2. Uniform Sales & Use Tax Certificate- Place your Sales Tax Registration Number in the box next to your state and Sign & Date the bottom.
