Agency for Healthcare Research & Quality Hospital Survey on Patient
Safety Culture
Recognizing the need of healthcare systems to be able to assess employee attitudes
about patient safety and quality improvements, the Medical Errors Workgroup of the Quality
Interagency Coordination Task Force (QuIC) sponsored the development of a survey
tool, the Hospital Survey on Patient Safety Culture. The tool will help hospitals
evaluate the level of employee support for safety and quality issues.
American Nurses Association - Nursing Safety and Quality Initiative
Nursing's Safety and Quality Initiative is developing clear direction
and focus for the practice of nursing in the future. The Initiative
has enabled ANA to articulate for the consumer and health policy maker
nursing's unique contributions to patient care and has begun to measure
nursing's impact on patient safety and quality of care.
American Society of Health-System Pharmacists' 2015 Initiative
In June 2001, ASHP created the ASHP Vision Statement for Pharmacy Practice in Hospitals and Health Systems.
Essential themes in the vision are that health-system pharmacists will help make medication use more
effective, scientific, and safe; and they will contribute meaningfully to public health. Based on the
vision and with significant input from members, ASHP has established some ambitious and measurable goals
and objectives for pharmacy practice in health systems to be achieved by the year 2015.

SafeMedication.com features complete, easy-to-read information on more than 700 drugs. It is
based on ASHP's premier drug information resources, which are developed independently by pharmacists and
other medication experts.
Joint Commission on Accreditation of Healthcare Organizations
JCAHO's 2006 National Patient Safety Goals
On May 20, 2005, the Joint Commission's Board of
Commissioners approved the 2004 National Patient Safety Goals (NPSGs).
National Coordinating Council for Medication Error Reporting and
Prevention Recommendations
The National Coordinating Council for Medication Error Reporting and Prevention is an independent
body comprised of 24 national organizations. In 1995, USP spearheaded the formation of the National
Coordinating Council for Medication Error Reporting and Prevention to meet, collaborate and cooperate
to address the interdisciplinary causes of errors and to promote the safe use of medications.
USP Patient Safety Recommendations
In pursuit of its mission to promote public health, USP establishes state-of-the-art standards
to ensure the quality of medicines for human and veterinary use. USP also develops authoritative
information about the appropriate use of medicines. National health care practitioner reporting
programs support USP's standards and information programs.
